ABVM applications will be accepted via the online process only and must be completed in one session.
All notarized and paper document requirements will need to be converted to electronic format. Be sure
you have all of the correct documentation before starting and upload it into the proper space. You cannot
upload more than one document into each space, and you cannot edit your application once it has been
submitted. Any application that is incomplete or needs revision, will be assessed a $50.00 charge. |

-Current State Medical License [If your state license expires before the exam dates, you should not submit your application until you have received your new license.
Your application will not be reviewed if you submit a license that expires before 9/10/10.]
-Primary Board Certification [If sitting for the general exam, you
must upload your Internal Medicine board certificate as your primary board. If
sitting only for the Endo exam, you may submit your Interventional
Cardiology board certificate. If sitting for both exams, please submit your Cardiovascular Disease certificate.]
ABVM General Exam:
-Vascular Medicine Fellowship Certificate
(Submit this document only if you have completed
12 months of formal training in vascular medicine).
-OR-
-Acceptable signatures: Program Director, Chief of Staff OR Department Chairman—no exceptions. Letters signed by nurses, etc. will not be accepted. The title of the physician signing your letter must be underneath the signature. If you are sitting for both exams, you cannot have the same physician write both letters for you. The notary seal must be visible or it will not be accepted.
ABVM Endovascular Exam:

Acceptable signatures: Director of Cardiac Cath Lab, Director, Interventional Radiology, Program Director or Chief of Medical Staff—no exceptions. The title of the physician signing your letter must be underneath the signature. Letters signed by nurses, etc. will not be accepted. If you are sitting for both exams, you cannot have the same physician sign both letters. The notary seal must be visible or it will not be accepted.
